Agreement Termination Request

    If your chapter or council has decided you no longer wish to participate in the SHRM Affiliate Website Program and have a SHRM hosted website, please have the current chapter president or state council director submit this form to terminate the agreement.

    To ensure you receive our reply, make sure your spam blocker accepts email from the shrm.org domain.

    The new website address you enter will be provided to your Field Services Team.  If you will no longer have a website, please enter NA.
    • The termination date entered above should be no more than 30 days from today
    • We process termination requests Monday - Friday. If the date you enter falls over the weekend, the request will be processed during the next business week.
